| What is the difference between Insurance and the AMG Plan? |
| The difference between the two is that the AMG plan is a medical coverage designed to include the services provided by our medical groups. The AMG plan does not cover services done outside our offices such as hospital services, specialist care and major surgeries. We offer discounts for MRI/CT scans, colonoscopies/endoscopies and medications. |

| Do you cover Specialist care? |
| Unfortunately, we do not cover any specialists. The only specialties we cover are Family Medicine, Pediatrics, Office-based Gynecology and Office-based surgery provided in our facilities. |

| Do you cover Dental? |
| No, but we have a partnership with Healthplex that can cover your dental needs with an additional cost. |

| Do you cover Vision? |
| No, we do not cover vision care. |

| Do you cover hospitalizations and major surgeries? |
| No, we do not cover hospitalizations and major surgeries but we do cover surgeries that can be done in our offices, such as lipoma removal, wart removal, excision of benign/malignant skin lesions, etc. |

| After signing up, how long does it take before I become effective and before I can make an appointment? |
| Membership starts on the first of each month. Therefore, the effective month is your choice depending on when you want to be seen. For example, if you want to be seen immediately your effective date will be the first day of that month. |

| Does the AMG Plan cover my medications? |
| No, we do not cover your medications but we do offer a discount on medications. We provide a two-tier level of discounts. One tier operates on the condition that if you take your prescriptions to any pharmacy, you will get up to 50% off on any generic drug and up to a 20% discount on brand-name drugs. The second tier offers a deeper discount through mail-order on generic drugs distributed by Health Warehouse. You can see the price list of these discounted drugs through Health Warehouse on our website. |

| What happens to my membership after the twelve months expire? |
| After you have honored the twelve-month agreement, we will continue to charge your account unless you notify us to terminate it. There is no termination fee after you have honored the twelve month agreement. |
